3. You understand that blood test results reported by LabTest@YourRequest.com will be reported directly to you. I will determine how my results are sent to me. I further understand that it is my responsibility to consult my own medical doctor for interpretation, explanation, analysis, and evaluation of my results.

I understand that neither LabTest@YourRequest.com, its shareholders, officers, Medical Directors, employed physicians, directors, or its other agents or employees shall not be liable for any claims including, but not limited to any claim arising out of or related to, un-interpreted, inaccurate, misinterpreted or results not received and do hereby expressly forever release and discharge all claims, damage, actions, demands, or causes of action.
    
5. I certify that I am not a recipient of Medicare, Medicaid, Tricare, or any other government health insurance benefits, nor will I seek to be reimbursed by Medicaid, Medicare, Tricare, Peachcare, Tencare, or any other government insurer and/or Payor for the performed test. I agree that I am financially, personally responsible for payment of all fees for all tests ordered and collected by LabTest@YourRequest.com.
    
6. I understand that the blood and/or urine tests performed by LabTest@YourRequest.com are done solely at y request to be screened through either blood and/or urine testing. I further understand that a physician employee of LabTest@YourRequest.com. Who is licensed under Georgia and Tennessee law to order such testing, will do so. I also understand that LabTest@YourRequest.com is a collection facility and that actual testing will be performed by a third-party laboratory certified to perform such testing on my urine and/or blood specimen collected by LabTest@YourRequest.com.
    
7. I understand and agree that LabTest@YourRequest.com. Will report the results of the testing directly to me, my physician, or any health professional I request. I consent and authorize that such disclosure may be made by mail, fax, or e-mail. I understand and agree that the services provided by LabTest@YourRequest.com. And the results from the lab will be maintained as confidential, protected health information by LabTest@YourRequest.com as required by federal and state law.
    
8. The tests, products, and/or services offered on our website are not intended to treat, cure disease, or diagnose and no warranty is given to our customers for the prevention of disease. Our services are not intended to substitute for a physician consultation.
    
9. Educational material is provided by LabTest@YourRequest.com to increase the knowledge of our customer base. This is to increase self-care and self-awareness. This will improve the patient and health care provider relationship. We encourage the patient to consult and work with their healthcare provider.
