Lab Test Process

1. Order online or call Lab Test @ Your Request.com at 1-866-873-0457 and
speak with our lab professionals for your request.

2. We will provide you with your orders, HIPAA release forms, requisition, and schedule an appointment near you. Your Physician's orders are also accepted.

3. Lab Test at Your Request.com accepts all major credit cards. Once you have placed an order, you will be emailed a confirmation to serve as a receipt. To receive our discounted rates payment must be made with Lab Test @ Your Request.com and You must have a Order Requisition form before services are rendered at the Patient Service Centers.

4. Our licensed professionals will honor your request by drawing a simple blood specimen to be tested at the laboratory.

5. Our licensed professionals will analyzed the specimen. Results are reviewed by our Board Certified Physicians and health care professionals

6. Your lab test request will be sent directly to you.
